The Canadian French edition of “Santé en milieu communautaire (SMC) interRAI, manuel de l'utilisateur et formulaire d'évaluation" is translated from the English "interRAI Community Health (CHA) Assessment Form and User’s Manual." This instrument was developed in response to user requests for a modular instrument that could efficiently assess the well elderly individual and identify those persons who merit further assessment in order to prevent or stabilize early functional or health decline. The instrument comprises a brief basic assessment and four supplements: fonctionnel (SMC-SF), santé mentale (SMC-SM), résidence avec services (SMC-RS), and pour sourd et aveugle (SMC-SA). The target population is adults living in the community and a variety of residential care settings.

This book contains introductory material; an item-by-item guide to the basic SMC assessment form; triggers and algorithms for the four supplements; item-by-item guides for each of the four supplemental assessment forms; the basic assessment form; and all four supplemental assessment forms. The Canadian French Edition has been customized for Canada, and the French language used in this publication is native to Canada.
