interRAI Community Health (CHA) Assessment Form and User's Manual, (Standard English Edition), 9.1.3
The interRAI Community Health (CHA) Assessment System was developed in response to user requests for a modular instrument that could efficiently assess the well elderly individual and identify those persons who merit further assessment in order to prevent or stabilize early functional or health decline. The CHA instrument comprises a brief basic assessment and four supplements: Functional (CHA-FS), Mental Health (CHA-MH), Assisted Living (CHA-AL), and Deafblind (CHA-Db). The target population for the CHA instrument is adults living in the community and a variety of residential care settings.
This book contains introductory material; an item-by-item guide to the basic CHA assessment form; triggers and algorithms for the four supplements; item-by-item guides for each of the four supplemental assessment forms; the basic assessment form; and all four supplemental assessment forms.
