The interRAI Pediatric Home Care Assessment (PEDS-HC) is a standardized assessment tool developed for use in programs serving children with special health care challenges. The PEDS-HC instrument is designed to be used to assess the home care challenges of children and youths ranging in age from 4 through 20 (in the USA) who are seeking or receiving long-term services or supports.
This book contains introductory material, an item-by-item guide to completing the assessment form, and the assessment form itself.
