The Canadian French edition of “Soins de longue durée en établissement (SLD) interRAI, manuel de l’utilisateur et formulaire d’évaluation" is translated from the English "interRAI Long-Term Care Facilities (LTCF) Assessment Form and User’s Manual." The SLD Assessment System is a comprehensive, standardized system for evaluating the needs, strengths, and preferences of persons in chronic care and nursing home institutional settings. Completing an SLD assessment enables a health care provider to assess key domains of function, mental and physical health, social support, and service use.
This book contains introductory material, an item-by-item guide for using the assessment form, and the assessment form itself. The Canadian French Edition has been customized for Canada, and the French language used in this publication is native to Canada.
About the Additional Materials
This book (published in 2021) includes additional material that was not part of the previous edition (published 2012). This additional material is an Addenda document that lists additions to the published LTCF Canadian French manual and assessment form, version 9.1.2.
