The interRAI Acute Care (AC) Assessment System was developed to identify older and disabled people admitted to acute hospitals with conditions that may benefit from comprehensive specialist assessment. It covers a wide range of clinical domains. Assessments are carried out on two or more occasions. Within 48 hours of admission, the individual's pre-hospital-admission status and status early in the hospital stay are recorded. Review assessments can be conducted during the stay when there is a significant change in status or when the person's hospital stay lasts more than 14 days. A discharge assessment is completed shortly before the person's anticipated discharge from the hospital. The target population is older or disabled people who have been admitted to any acute hospital service for inpatient treatment and for whom comprehensive assessment may be helpful. The instrument is suitable for acute geriatric assessment units and geriatric consultation services; it is also suitable for targeted use in general medical and surgical wards.
